The Heart Failure Center Nurse Practitioner (APNP) provides ongoing care regarding heart failure disease state management under the supervision of the HFC Medical Director. The primary responsibilities of the APNP are assessment, evaluation and education, implementing the plan of care, and counseling for each patient. The scope of accountability includes clinical practice utilizing evidence based practice. The APNP is responsible for program coordination and collaboration, and over sight of medication reconciliation. The APNP must remain current in their understanding of American Heart Association heart failure clinical guidelines and recommendations, including a good understanding of cardiac and heart failure physiology and patient management. The APNP is able to function independently and in collaboration with a multidisciplinary team. This includes the patient's PCP, specialists, dietician, pharmacist, palliative care team, and HFC support staff.

- Master's degree in Nursing
- Current Wisconsin RN Licensure
- Nurse Practitioner Certification by a national certifying body
- Advanced Practice Nurse Prescriber Certification
- CPR certification
- Proficient in health information technology, as well as computer usage
- Three years of nursing experience required, preferably with older patients who have chronic conditions, specifically heart failure.
