Legacy Care, a fast growing division of LGS Medical Services Group, is seeking experienced Nurse Practitioners (NP) interested in working with this Richmond, VA-based medical group specializing in skilled rehab, nursing home and assisted living care.

Legacy Care, a group of over 40 providers in the Richmond, Charlottesville, and Fredericksburg, VA areas, is expanding to the Winchester, Harrisonburg, Roanoke and Williamsburg, VA areas. We currently have a need for NPs who are able to work independently and collaboratively with a provider team to deliver high quality care to residents in Inpatient Rehabilitation, Post-Acute and Long Term Care. We are seeking NPs who appreciate their patients, excel in a collaborate environment, and strive for positive outcomes for their patients.

This position involves care for inpatient rehabilitation including long-term and skilled/short term rehab residents. You will be part of a provider team and assume care of high risk, frail patients with multiple comorbidities among a team of PT, OT, speech, case management, nursing, and medical staff. Collaboration and communication with team physician and fellow NPs is essential, however must be capable and willing to work independently and assume care of facility residents. Some call will be required.

Competitive compensation can range from $120,000 to $200,000 based on productivity.This is an ideal position for someone who is independent, efficient, productive, and desires to be self-motivated with the benefit of provider support. New NPs with experience are welcome to apply.


  • Graduate of an accredited NP program required;
  • Must possess, or be able to obtain, a Virginia NP license;
  • Geriatric experience a plus;
  • Very strong customer service orientation, positive attitude, highly self-motivated, directed and change oriented;
  • Must be able to speak English clearly and fluently;
  • Must be able to read and interpret clinical data;
  • Professional appearance.
