Nurse Practitioner for Geriatric Home Health Assessments

The position can be either full time, part time or per diem. Nurse practitioner is paid per assessment, paid for training, and reimbursed for all travel
Nurse Practitioner needed to conduct in home assessments for assigned members of INSPIRIS.
INSPIRIS is a healthcare management company focused on improving the quality of life for the elderly population, while reducing the cost to Medicare Advantage health plans. They partner with individual health plans to provide acute, post-acute, custodial and hospice-care programs that address the needs of the plan's membership. The health assessments are to update the health status of the member and reduce the number or hospitalizations and ER visits.
INSPIRIS offers paid training and travel reimbursement. Minimum of 2 days/week required. Provider (NP or PA) chooses own schedule and territory, nights and weekends allowed.
Assessments Program is a targeted, multi-dimensional in-home evaluation that accurately reflects the health status of a member. A comprehensive clinical service with significant financial impact, the program is designed to meet CMS and state requirements for risk coding, clinical assessment, and regulatory compliance. Our prospective assessment is unique in that it is not only a risk-adjustment management solution but it also offers a health plan the unparalleled ability to address pertinent gaps in care delivery for members participating in the program.
The provider (NP) visits the member at their residence to conduct a comprehensive medical evaluation that captures all of a member’s health characteristics and provides a complete picture of their health status.
Our Targeted Visit Includes:
• A medical history review and analysis of available medical records
• Assessment of caregiver/family support
• Multi-dimensional needs assessment, with specific attention to physiological,
psychosocial, behavioral, functional and environmental needs
• Diagnosis capture
• Encounter data and transmission via appropriate manner

Full time or part time, it requires a commitment of at least 2 days a week.
Travel required, not extending beyond a 30 mile radius
Territory and work schedule based on location and availability of the Nurse Practitioner
