Position Description

POSITION SUMMARY

The Adult or Family Nurse Practitioner functions as a member of a medical team and applies to the treatment of patient's the medical and related skills acquired through the training and/or experience required for this position. S/he may provide services within one or more Wheeler Clinic program(s), and the relative concentration between and within programs may change with the needs of the Clinic.  S/he will see on average, 15 patients, in a 8-hour day.

Other staff include: Psychiatric Nurse Practitioner, Primary Care Nurse Practitioner, Medical Assistants, RNs, front desk staff and behavioral health staff.

Full Time salary of $115,000-$135,000 based on years of experience.

LOCATION
Waterbury, CT

SCHEDULE

  • Full Time, 40 hours per week.
  • Five 8 hour days including one evening per week
  • Rotating Saturday coverage based on program needs (9am-1pm)
  • Rotating on call required

QUALIFICATIONS

  • Licensed as an Advanced Practice Registered Nurse (APRN)
  • 2 years of relevant work experience
  • Board Certified as an Adult or Family Nurse Practitioner   
  • Active DEA Controlled substance registration
  • Must have the suboxone waiver

ADDITIONAL POSITION REQUIREMENTS

  • Will be required to acquire and maintain certification in Basic Life Support for Healthcare -Professionals no later than 90 days from date of hire.
  • Must be able to pass a respirator fit test at time of hire and on an annual basis.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Conducts medical evaluations and formulates accurate diagnoses in accordance with the International Classification of Diseases.  Manages prescribed medication, performs physical health examinations, conducts preventive screening and care, develops and presents clear clinical impressions and recommendations that are specific and attainable.  Develops and documents clear clinical formulations and medical treatment plans.

Prescribes therapeutic interventions appropriate to identified problems.  Administers drugs and biologicals as required by the Clinic. 

Demonstrates knowledge of physical health concerns and effective medical interventions. Triages clinical emergencies and implements safety and consultation protocols.  Utilizes evidence based and best medical practice.

Monitors patients' self-care management and self-care skills and provides prevention, wellness, and pertinent medical education. 

Provides requisitions for clinical studies, diagnostic tests, and laboratory studies.  Evaluates and interprets results to identify and assess patient's clinical problems and health care needs.

Demonstrates knowledge of internal and external levels of care, referral processes and resources. Makes clinically appropriate referrals for consultation with specialists, subspecialist, and allied health professionals as indicated.

Works with care coordination team in designing, monitoring and implementing care plans.  Communicates referrals and collaborates on development of services planning and implementation.
