Position Description

Felton Health Care Specialists is seeking a full-time, board-certified ARNP with experience in Gerontology, Adult Primary Care, or Acute Critical Care Medicine to join our growing team.

Position Description

Experienced ARNP will care for complex patients in adult family homes/independent living/assisted living/skilled nursing facilities/memory care facilities for both acute and chronic care management.

The Nurse Practitioner works with patients, families, facility staff and community partners, including Hospice and Home Health providers, to establish and manage care plans through comprehensive knowledge of acute and chronic conditions, diagnostics and treatment related to long term care.

The Nurse Practitioner will assess patient conditions, educate patients and family members about medical options, prescribe medications and treatments, and perform other duties to support patient care. The Nurse Practitioner may also provide continuous education to facility staff for the purposes of continuous quality improvement.

Gerontological Nurse Practitioner Job Responsibilities

  • Provide medical care for patients
  • Plan and implement individualized plans of care for patients
  • Order and interpret diagnostic tests as needed
  • Formulate diagnoses and treatments then evaluate response to therapy
  • Ensure that prescriptions and durable medical equipment are ordered in a timely manner
  • Maintain and share current patient records in a HIPPA compliant fashion
  • Provide education, counseling and emotional support for patients and family members
  • Facilitate patient and family decision making regarding rehabilitation, long term care, life support, end of life care and other aspects of advance care planning
  • Collaborate with facility staff to minimize risk and improve patient outcomes

Minimum Qualifications:

  • Master's degree or Doctorate
  • Washington State Nurse Practitioner Licensure
  • Current certification by the American Nurses Credentialing Center (ANCC) or National Certificate Corporation (NCC)
  • DEA license
  • Individual Professional Liability Insurance
  • Understanding of diagnostic tests and ability to interpret test results
  • Relevant continuing medical education
  • References attesting to knowledge of acute and chronic conditions and treatments of geriatric patients
  • A minimum of two years of experience preferred, but new NP graduates with geriatric nursing, acute critical care or long-term care RN experience are welcome to apply.

We provide patient care for those residents who prefer an on-site primary care medical provider. We see individuals routinely every two to three months, or as needed, where they live. We are available 24/7 by phone to patients, families and caregivers.

We also provide medical directorship for the facilities. We serve as part of the leadership team to improve health outcomes and quality of life for residents by offering medical direction related to health care of residents, education of staff and policies and procedures for the facility.
