Position Description

Alamo Hospice is adding to our team of Full Time and Part Time Nurse Practitioners to cover the San Antonio and the Texas Hill Country area.

Part-Time:

Requires 3 days/week plus one meeting on the first or second Tuesday each month from 5-6:30 at our San Antonio office. Covers the San Antonio and the Texas Hill Country area.

Full-Time

Requires Monday - Friday plus one meeting on the first or second Tuesday each month from 5-6:30 at our San Antonio office. Covers the San Antonio and occasionally the Texas Hill Country area.

Job Description:

A licensed and credentialed Nurse Practitioner provides diagnosis, treatment, consultation, and follow-up (depending on the state licensure) under the supervision of 1 or more physicians in the practice. The Nurse Practitioner provides age and specialty appropriate medical care at the level of training achieved. The Nurse Practitioner should at a minimum assess: Disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events, the impact of the disease burden on the patient’s condition on the patient, caregiver and/ or family’s quality of life. 

Our goals are to care for end of life patients with a life-limiting illness with a prognosis of 12 months or less. We become the liaison between all of their different medical providers with the goal of keeping them out of the hospital. They can still pursue curative/life-prolonging treatments while on palliative which include chemo, dialysis, home health, etc. Demonstrates excellent observation, assessment, nursing judgment and communication skills. Ability to provide accurate written documentation in a timely manner. Self-directed with the ability to work with little supervision. Flexible and cooperative in fulfilling all obligations.

Required Skills:

  • Ability to work within an interdisciplinary setting.
  • Understanding of Hospice philosophy, principles of death/dying.
  • Compliance with accepted professional standards and practices.
  • Valid driver’s license and reliable transportation that is insured in accordance with state and/or company's requirements.
  • Demonstrate excellent observation, assessment, nursing judgment and communication skills.
  • Ability to provide accurate written documentation in a timely manner.
  • Self-directed with the ability to work with little supervision.
  • Flexible and cooperative in fulfilling all obligations.

Required Experience:

  • Graduate from an accredited school of nursing and certified as a Nurse Practitioner.
  • Masters of Science in Nursing.
  • Nurse Practitioner specialty in Family, Adult, Geriatric or Acute Care preferred.
  • A minimum of two years Nurse Practitioner experience is required.
  • Current Hospice or home health experience with pain and symptom management interventions preferred.
