Position Description

Advanced Heart Failure and Cardiac Transplant Physician Assistant or Nurse Practitioner 

Dartmouth-Hitchcock Clinic and the Heart and Vascular Center is seeking an advanced practice provider (APP: nurse practitioner, physician assistant) with a strong interest and/or background in advanced heart failure and transplant cardiology. This individual will work with the multidisciplinary, advanced heart failure team, providing inpatient and outpatient care to complex patients with advanced heart disease. 

In this role, the APP would function as the primary provider for both inpatient (consultation) and outpatient (clinic-based) care. Consultation services are provided for intensive care units, general cardiology services and medicine services. In this role, the APP would function under the direct supervision of an advanced heart failure and transplant cardiologist. Outpatient care is largely provided independently by the APP, with the attending physician always available. The scope of practice also includes a teaching role and participation in quality improvement activities. APPs play an important role in clinical teaching and a faculty appointment through Geisel School of Medicine at Dartmouth will be requested.

Prior experience and/or interest in invasive hemodynamic monitoring (pulmonary artery catheters), temporary (IABP, Impella, RP Imeplla, Centrimag and ECMO support) and durable (HeartMate 2, HeartMate 3 and HVAD) mechanical circulatory support (MCS) devices and/or cardiac transplantation is preferred. If the applicant does not have prior experience with these, in-house and/or external training will be provided. Prior experience and/or interest in pulmonary hypertension, cardiopulmonary exercise testing, IV infusions, CardioMEMS and/or palliative care would also be assets. 

The advanced heart failure/transplant team is a multidisciplinary team comprised of attending physicians, 2 APPs, 2 care management nurses, a triage nurse, 2 administrative assistants and a clinical nutritionist. This position is would be full time, Monday through Friday during normal business hours (approximately 8am until 5pm) with holidays and weekends off.

Graduate of an accredited PA academic program
Certification through the National Commission on Certification of Physician Assistants (NCCPA)
Eligible for PA licensure in the State of NH
Eligible for Federal DEA Registration
BLS certification

APRN Minimum Qualifications/ Required Licensure/Certification Skills:

Graduate of an accredited APRN academic program
National specialty certification as FNP, Acute Care ANP or Primary Care ANP
Eligible for RN licensure in the State of NH
Eligible for APRN licensure in the State of NH
Eligible for Federal DEA Registration
BLS certification
