Position Description

Advanced Practice Clinician (NP or PA) – Senior Living Program

Schedule: Full-time, 40 hours/week

Type: Temp-to-Hire Opportunity

Location: Multiple senior living facilities across Clackamas, Multnomah, Washington, Clark, and Cowlitz counties in Oregon

Shift: Monday–Friday, 8:00 AM – 5:00 PM

Travel: Required between facilities during shift

Dress Code: Medical attire

About The Role

Join a collaborative care team dedicated to improving health outcomes for seniors in a facility-based setting. This position offers work-life balance with flexible scheduling, no on-call, no weekends, and no holidays. You’ll provide integrated, longitudinal care to high-risk members as part of an interdisciplinary team.

Key Responsibilities

  • Perform comprehensive assessments and manage acute/chronic conditions
  • Document accurately using ICD-10 codes
  • Collaborate with interdisciplinary teams
  • Lead advanced illness and care planning discussions
  • Educate patients and caregivers on health management

Required Qualifications

  • Active, unrestricted NP or PA license in Oregon (or ability to obtain)
  • NP: MSN or DNP from accredited program; Board certification (AANP or ANCC) in FNP, Adult, Gerontology, or Adult-Gerontology Acute Care
  • PA: Degree from accredited PA program; NCCPA certification
  • Current DEA license/prescriptive authority
  • Ability to meet physical requirements (lifting 30 lbs, navigating stairs, kneeling for assessments)
  • Reliable transportation
  • Full COVID-19 vaccination

Preferred Qualifications

  • 1+ year clinical experience (long-term care preferred)
  • Experience with complex behavioral, social, and functional needs
  • Knowledge of geriatrics, chronic illness, and end-of-life care
  • Strong EMR documentation skills
