Advanced Practice Clinician – Nurse Practitioner / Physician Assistant

About The Job

The Senior Community Care (SCC) program is a longitudinal, integrated care delivery model that coordinates clinical care for members in facility settings. As an Advanced Practice Clinician (APC) within SCC, you will provide care to our highest-risk health plan members as part of an interdisciplinary team. This role offers flexible scheduling with no on-call, weekends, or holidays required, allowing for excellent work-life balance. Responsibilities

  • Conduct comprehensive patient assessments.
  • Manage acute and chronic medical and behavioral conditions.
  • Ensure accurate and complete documentation, including ICD-10 coding.
  • Communicate and collaborate with the interdisciplinary care team.
  • Lead advanced illness and care planning conversations.
  • Provide counseling and education to patients and caregivers.

Minimum Qualifications

  • Active, unrestricted license in your state of residence (or ability to obtain by start date).
  • Education:
    • NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
    • PA: Graduate of an accredited Physician Assistant degree program.
  • Active national certification:
    • NP: Board certified through AANP or ANCC in one of the following:
      • Family Nurse Practitioner
      • Adult Nurse Practitioner
      • Gerontology Nurse Practitioner
      • Adult-Gerontology Acute Care Nurse Practitioner
    • PA: Board certified by NCCPA.
  • Current DEA licensure/prescriptive authority.
  • Ability to meet physical requirements (lifting 30 lbs, navigating stairs, performing assessments).
  • Reliable transportation.
  • Ability to obtain collaborative practice agreement if required by state.
  • Full COVID-19 vaccination (per federal, state, and client requirements).
  • Must pass drug screening prior to employment.
Preferred Qualifications

  • 1+ year of clinical experience (long-term care preferred).
  • Experience managing patients with complex behavioral, social, and functional needs.
  • Knowledge of geriatrics, chronic illness, and end-of-life care.
  • Proficiency in electronic medical records and documentation.

Additional Details

  • Duration: Approx. 6 months
  • Schedule: Monday–Friday, 8:00 AM – 5:00 PM
  • Dress Code: Medical attire (scrubs color: N/A)
  • Travel: Morris and Union Counties, NJ (during shift)
