Responsibilities
- Conduct comprehensive patient assessments.
- Manage acute and chronic medical and behavioral conditions.
- Ensure accurate and complete documentation, including ICD-10 coding.
- Communicate and collaborate with the interdisciplinary care team.
- Lead advanced illness and care planning conversations.
- Provide counseling and education to patients and caregivers.
- Active, unrestricted license in your state of residence (or ability to obtain by start date).
- Education:
- NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- PA: Graduate of an accredited Physician Assistant degree program.
- Active national certification:
- NP: Board certified through AANP or ANCC in one of the following:
- Family Nurse Practitioner
- Adult Nurse Practitioner
- Gerontology Nurse Practitioner
- Adult-Gerontology Acute Care Nurse Practitioner
- PA: Board certified by NCCPA.
- Current DEA licensure/prescriptive authority.
- Ability to meet physical requirements (lifting 30 lbs, navigating stairs, performing assessments).
- Reliable transportation.
- Ability to obtain collaborative practice agreement if required by state.
- Full COVID-19 vaccination (per federal, state, and client requirements).
- Must pass drug screening prior to employment.
- 1+ year of clinical experience (long-term care preferred).
- Experience managing patients with complex behavioral, social, and functional needs.
- Knowledge of geriatrics, chronic illness, and end-of-life care.
- Proficiency in electronic medical records and documentation.
- Duration: Approx. 6 months
- Schedule: Monday–Friday, 8:00 AM – 5:00 PM
- Dress Code: Medical attire (scrubs color: N/A)
- Travel: Morris and Union Counties, NJ (during shift)
