Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Dress Code: Medical attire (no specific scrub color required)

Role Overview

The Advanced Practice Clinician (APC) will support the Senior Community Care (SCC) program, a longitudinal, integrated care model focused on high-risk members residing in facility settings. This role is part of an interdisciplinary team delivering comprehensive clinical care and care coordination.

Responsibilities

• Conduct comprehensive assessments across medical and behavioral domains
• Manage acute and chronic conditions effectively
• Ensure accurate documentation including ICD-10 coding
• Collaborate with interdisciplinary teams to coordinate care
• Lead advanced illness and care planning discussions
• Educate and counsel patients and caregivers
  
  

Required Qualifications

• Active, unrestricted NP or PA license in practicing state (or ability to obtain by start date)
• NP: Graduate of accredited MSN or DNP program
• PA: Graduate of accredited PA program
• Board certification:
• NP: AANP or ANCC in one of the following:
• Family Nurse Practitioner
• Adult Nurse Practitioner
• Gerontology Nurse Practitioner
• Adult-Gerontology Acute Care Nurse Practitioner
• PA: NCCPA certification
• Current DEA license / prescriptive authority
• Ability to meet physical demands (lift 30 lbs, navigate stairs, kneel, etc.)
• Reliable transportation
• Collaborative practice agreement (if required by state)
• Fully vaccinated for COVID-19 (per UHG policy and regulations)
• Must pass pre-employment drug screening
  

Preferred Qualifications

• 1+ year clinical experience (long-term care preferred)
• Experience with complex patient needs including behavioral and social factors
• Knowledge of geriatrics and chronic illness
• Comfort with end-of-life discussions
• Proficiency in EMR documentation
