Wound Care Specialist

Reports To: Vice President of Clinical Services (or Clinical Leadership)

Work Setting: Clinics, Skilled Nursing Facilities, and Home Health Environments

Position Summary

The Wound Care Specialist is responsible for providing high-quality, patient-centered wound care across multiple clinical settings. This role includes assessing, managing, and treating patients with acute and chronic wounds, performing debridement procedures, and developing individualized treatment plans. The ideal candidate demonstrates strong clinical judgment, compassion, and a commitment to improving patient outcomes.

Essential Responsibilities

• Perform comprehensive physical exams and wound evaluations.
• Assess wound conditions, review comorbidities, and coordinate care with managing providers.
• Conduct wound debridement, dressing changes, and other clinically necessary procedures.
• Develop and implement individualized treatment plans with interdisciplinary teams.
• Complete all clinical documentation within 24 hours in the EMR system.
• Provide patient and caregiver education on wound management and preventive care.
• Communicate effectively with primary care providers, specialists, and care teams.
• Collaborate with clinical support staff to ensure workflow efficiency.
• Participate in clinical meetings, trainings, and quality improvement initiatives.
• Advocate for patients and ensure access to appropriate medical resources and follow-up care.
• Maintain compliance with clinical, safety, and regulatory standards.
• Maintain active licensure and ongoing continuing education in wound care or related specialties.
• Stay current with CMS documentation and treatment guidelines.
• Perform additional duties as assigned.
  
  

Qualifications

Required

• Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in applicable state(s).
• Clinical experience in wound care or a related specialty.
• Proficiency in wound assessment, debridement, and developing care plans.
  
  

Preferred

• Wound care certification (WCC, CWCN, or equivalent).
• Experience in SNFs, home health, or outpatient wound care programs.
• Familiarity with EMR systems and mobile charting tools.

Work Environment

• Field-based role with regular travel between patient care locations.
• Frequent standing, walking, and movement throughout care sites.
• May involve exposure to infectious or complex conditions; PPE provided.
• Role may include lifting up to 25 lbs.
