The 24 waiver training for buprenorphine is NOW AVAILABLE and the SAMHSA waiver application is NOW FIXED (thanks in large part to Jose!!), so we need to spread this information out to our organizations!!

Once the 24 hours is completed, the NP or PA needs to fill out the waiver application at:
http://buprenorphine.samhsa.gov/forms/select-practitioner-type.php

And then email certificates and DEA and licenses to:
NPs and PAs may send copies of their training certificates to infobuprenorphine@samhsa.hhs.gov or faxed them to 301-576-5237
