Referring to WIC Just Got Easier!

There is a new Colorado WIC Referral Form

found at: www.coloradowicsignup.com

This will make referrals easier for the provider! It is a short, online form health care providers and community programs can complete on behalf of a patient/client. The form can also be submitted by families/individuals. WIC will take it from there!

The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) provides nutrition education, breastfeeding support, healthy food, and health referrals to Colorado families participating in Medicaid, SNAP or TANF or whose income is less than 185% of the Federal Poverty Level. WIC’s goal is to help keep pregnant and breastfeeding women and children under age 5 healthy, and improve long-term health outcomes.
