Dear Members,

Ms. Sarah Herbert, a future colleague and Family Nurse Practitioner student with the Orvis School of Nursing, is conducting a research study to learn about the process of assessment for the Patient Health Questionnaire 9 and modified 9A (PHQ-9 or PHQ-9A) depression screening tool for adolescents ages 12-17 years old in the state of Nevada. 

Please consider completing a short, 16 question, a survey regarding the identification and treatment of depression in adolescents and young adults.  The purpose of the survey is to learn how providers in Nevada are assessing depression in this particular patient population and if there is a consistent manner in which it is being performed.  Completion of the survey should take approximately 3-5 minutes; the link is included below.
