For those of you that need to apply to prescribe schedule II drugs in hospitals or hospice settings, here is the process for you to get that authority.

APRNs who have DPS numbers already for III through V can modify their registrations to add schedule IIs here: http://www.txdps.state.tx.us/RSD/ControlledSubstances/csforms.htm using the modification of registration form. 

The order form for the controlled prescription pads can be found here: http://www.txdps.state.tx.us/rsd/PrescriptionProgram/index.htm. 

If you don't already have a DPS registration, you will need to start from scratch and apply.
