-------------------------------------------
Greetings.... Please find below the link to the article in Pediatrics about the revised recommendations for pediatric preventive health care for 2014. These revisions provide guidance for PNP's in any setting (primary care, specialty care, and acute care) to help assure that all children receive recommended preventive health care screenings and services. These recommendations are considered the "gold standard" under the Accountable Care Act."

Because children with special health care needs often see their specialty providers more often than their primary care provider, or are frequently in the hospital more often than they see their primary care provider, it is important for all pediatric nurse practitioners to be aware of these recommendations and to work together to assure that the children they care for receive comprehensive pediatric health care.

As the Pediatric Health Care Home SIG we work to assure continuity of care between different clinical sites (primary care, acute care, and specialty care) especially for children with special health care needs. We welcome your suggestions, models of care, and other suggestions to share with other colleagues. As pediatric nurse practitioners we know the importance of communication among health care providers and how assuring smooth transitions help families provide better care for their children. Through our Pediatric Health Care Home SIG we will continue to learn from one another about how we can improve our practices on behalf of families.

If you are attending the national conference, please stop by the SIG Showcase and the Pediatric Health Care Home booth to say hello and learn more about our efforts. Barbara
