Complement-Mediated Thrombotic Microangiopathy (CM-TMA)—also known as atypical HUS

Complement-Mediated Thrombotic Microangiopathy (CM-TMA)—also known as atypical HUS sounds complicated and foreign to many, however all of us have probably seen it in our practice. Corby Lust has become an expert in this compliment related subject and will make it easy to identify patients at risk including patients with cerrtain triggers:
- Microangiopathic hemolytic anemia (MAHA): schistocytes, elevated LDH, low haptoglobin
- Thrombocytopenia
- Acute kidney injury (AKI) or rising creatinine
Consider
- Autoimmune disease (e.g., SLE): ANA, dsDNA, complement levels
- Pregnancy/postpartum
- Transplantation
- Infections (e.g., COVID-19, influenza)
- Malignancy
- Medications (e.g., calcineurin inhibitors)
If TMA persists after treating the primary condition, CM-TMA should be strongly considered.
