Position Description
GHR is seeking Nurse Practitioners and Physician Assistants for a top client in the Newport, RI territory! This role is responsible for delivering compassionate and holistic clinical services to patients within their homes, fostering meaningful relationships with patients and caregivers, and driving patient-centered strategies to enhance care outcomes. The Advanced Practice Provider will play a pivotal role in their value-based clinical model, focusing on patients with complex needs who are underserved by traditional healthcare systems.
Responsibilities:
- Provide individualized and comprehensive patient care, including urgent care, chronic condition management, and transitions of care, in alignment with our value-based care model.
- Collaborate with the multidisciplinary team to deliver social determinants of health interventions, behavioral health management, and complex medication oversight.
- Deliver exceptional clinical care through in-person and virtual settings, encompassing private homes, congregate living facilities, clinics, and skilled nursing facilities.
- Incorporate company performance goals, operational processes, and data dashboards into daily clinical practices.
- Actively engage in patient experience initiatives, ensuring patients are well-informed and empowered in their care journey.
- Maintain accurate medical records, complete clinical documentation, medication reconciliation, and communication logs.
- Participate in care team meetings, huddles, and partnership sessions, contributing to coordinated care and goal planning.
- Foster collaboration with internal and external care providers to ensure seamless care coordination and goal achievement.
- Adhere to medical policies, protocols, clinical guidelines, and compliance standards.
- Serve as a growth ambassador by building relationships with patients, caregivers, facility staff, and clinical partners.
- Participate in after-hours on-call schedules and cross-coverage when required.
- Undertake market-specific tasks as assigned.
Qualifications:
- Nurse Practitioner or Physician Assistant with accredited health professions school graduation and national board certification.
- Active licensure in Rhode Island.
- Active DEA licensure and state-specific controlled substance registration, as applicable.
- Advanced training in geriatrics and/or palliative medicine (preferred).
- Certification in basic life support (BLS) required.
- Prior clinical experience caring for aging adults and complex care patients.
- Experience in home settings (private homes and/or congregate living facilities preferred).
- Familiarity with managed care and value-based healthcare delivery (preferred).
- Proficiency in computer software and technology.
- Ability to travel as needed, with valid liability and property damage insurance on personal vehicle.
Driver Requirements:
- Minimum 3 years of licensed driving experience.
- No undisclosed violations or accidents within the last 3 years.
- No vehicle-related suspensions/reinstatements.
- No DUI, reckless driving, or felony convictions in the past 7 years.
