Your Role with the Company:

• Full time, 40 hours a week, no call
• FNP or PA will see all ages with a high elderly population
• APP will see on 16-22 patients per day
• Provider should be experienced in chronic disease management

Background Profile:

• Board Certified Family Nurse Practitioner or Physician Assistant
• FNP or PA with 2+ years experience in outpatient primary care
