Job/Practice Description:

• Full-time, 40 hours/week
• APP will support a busy Internal Medicine team seeing adult/geriatric patients
• Nurse Practitioner will provide the co-management of chronic illnesses and conduct annual wellness visits as needed
• The provider will join a busy primary care clinic and should be comfortable and confident caring for complex and co-morbid patients 

Background Profile:

• Board Certified Family Nurse Practitioner, Adult Geriatric Nurse Practitioner, or Physician Assistant
• 1 + years of experience working in primary care as an Advanced Practice Provider
