Position Overview:
The Palliative Nurse Practitioner collaborates with healthcare providers, staff at skilled nursing and assisted living facilities, as well as home health and hospice agencies. Under the supervision of physicians, the NP will provide palliative care to new and established patients in their homes or facilities through in-person and telehealth visits.

Key Responsibilities:

• Conduct comprehensive patient histories and physical examinations for palliative patients.
• Order and interpret laboratory, radiological, and diagnostic tests based on patient demographics and conditions.
• Diagnose and develop appropriate treatment plans within the scope of practice.
• Document visits within 48 hours to support medical billing and coding requirements.
• Educate and guide patients and families regarding medical conditions and care plans.
• Report significant patient changes to the attending physician and document updates in the EMR.
• Implement emergency measures or stabilization procedures when necessary.
• Participate in ongoing professional education and contribute to the training of students and staff.
• Actively engage in patient safety, quality improvement, and patient satisfaction initiatives.
• Perform specialty-specific procedures as outlined in the job agreement.
• Provide on-call coverage for medical and palliative care patients.
• Complete other assigned duties as needed.

Qualifications:

• Education: Master’s or Post-Master’s degree from an approved State Board of Nursing program.
• Experience: 1-3 years of Nurse Practitioner experience in geriatrics, palliative care, or hospice care.
• Licenses/Certifications:
  • Prescriptive authority in Pennsylvania 
  • Current CPR certification
  • Active RN and Nurse Practitioner license for PA 
  • National Board Certification through AANP or ANCC
  • DEA registration
