Nurse Practitioners (Geriatric Primary Care)

Overview:

• Value Based Care Model (allotting 45 min with each new patient!), Serving the senior population. Goal is for providers to build relationships with patients. Work alongside a doctor.
• Prescribe Medicine and work through non-complex cases
• Because of the Value Based Care model, the whole goal is to keep these patients out of the ER, so the org does a lot of tracking and monitoring the patients for check up appointments, scans, and preventative care (this leads to more admin work on NP side compared to other roles, especially the fee for service model)
• Depending on the market, could see walk ins
• Could do telehealth
• Admin
• EMR: eClinical Works

Experience Needed:

• Generally looking for NPs with 2-3 years of experience
• Experience working with the elderly patient population
• Have hired from multiple specialties (IM, family, geriatrics)
• Managed Care Experience is great

Nice to Have Skills

Experience working with a geriatric population, NP experience working within Integrative Medicine, Family Practice, Geriatrics, or Pain Management, Managed Care, Bilingual, Value Based Care Experience

Soft Skill/Attribute Requirements

• Soft Skills: Needs someone who has a passion for work - important that they understand the population that they will serve. Someone that's flexible and desire to provide care to senior population. Needs patience - balanced approach to care and able to spend more time with patient/build more rapport and relationship. Want to be one stop shop for all of their medical needs and keep them out of the ER. Allowing ample time for comprehensive assessments to ensure they are gathering data on any issues/historical issues.
