The Judge Group is seeking Full Time Nurse Practitioners in Maricopa, Arizona!

About the Role

We’re looking for a compassionate and highly skilled Nurse Practitioner (NP) to provide comprehensive healthcare services to patients throughout their healthcare journey. As an NP, you will conduct in-home visits and work in community-based settings such as assisted living facilities, skilled nursing facilities, and group homes.

Responsibilities

As a Nurse Practitioner, you will:

  • Conduct comprehensive in-home assessments (minimum of 25 per week), including medical history reviews, physical exams, and medication management.
  • Perform thorough health assessments, including physical exams, medical histories, and risk evaluations.
  • Diagnose and treat medical conditions using evidence-based guidelines.
  • Manage chronic diseases like diabetes, hypertension, and cardiovascular conditions, focusing on patient education and disease control.
  • Prescribe and manage medications, educating patients about proper administration and potential side effects.
  • Promote preventive care, including immunizations, screenings, and lifestyle modifications.
  • Educate patients on health conditions, self-care practices, and lifestyle improvements.
  • Collaborate with physicians, nurses, and healthcare professionals to ensure coordinated patient care.
  • Serve as the primary care provider of record for assigned patients.
  • Maintain accurate and timely electronic medical records (EMR), ensuring documentation is completed within 48 hours of each visit.
  • Build strong relationships with patients and their families, demonstrating empathy and excellent bedside manner.
  • Conduct follow-up visits to monitor treatment effectiveness and make adjustments.
  • Participate in quality assurance programs, complying with healthcare regulations and standards.
  • Provide after-hours on-call support, as needed.
  • Travel to member appointments and ensure reliable transportation.

Qualifications

To be successful in this role, you’ll need:

  • Active, unrestricted Nurse Practitioner licensure in the state where you’ll provide care.
  • Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner.
  • Autonomous provider status (required within six months of eligibility).
  • Minimum two years of experience in primary care or longitudinal care (preferred).
  • Active Drug Enforcement Administration (DEA) license.
  • Basic Life Support (BLS) Certification for healthcare providers.
  • Strong clinical assessment and diagnostic skills.
  • Excellent communication and patient education abilities.
  • Proficiency in electronic medical records (EMR) systems.
  • Reliable transportation and valid proof of car insurance coverage.
  • A valid driver’s license with a minimum of 3 years of driving experience without restrictions.
  • Ability to pass a background check and pre-employment drug screening.

Preferred Qualifications

  • Experience with HEDIS/STAR measures.
  • Prior home-based visit experience.
  • Familiarity with Hierarchical Condition Categories (HCC) documentation.

Physical & Work Conditions

  • Ability to lift and carry up to 20 lbs.
  • Comfortable sitting, standing, bending, and kneeling to perform assessments.
  • Ability to navigate stairs and different living conditions during home visits.
  • Work is primarily conducted in patient homes with potential exposure to environmental conditions.
