Title: Advanced Practice Clinician - Nurse Practitioner or Physician's Assistant

Location: Watertown, NY

Job Type: 52 Weeks Contract

Shifts: Monday – Friday, 8AM - 5PM

Pride Health is hiring an Advanced Practice Clinician - Nurse Practitioner or Physician's Assistant to support our client's senior community care program in the region of New York. This position will float to 2 facilities to cover patient care needs.

Summary:

The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. The APC within SCC provides care to our highest-risk health plan members and is part of an interdisciplinary team.

Primary Responsibilities:

• Conduct comprehensive assessments
• Effectively manage acute and chronic medical and behavioral conditions
• Ensure accurate and complete documentation, including ICD 10 conditions
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide patients and caregivers with counseling and education

Required Qualifications:

License: Active and unrestricted (or ability to obtain)

Education:

• NP: Master's or Doctorate in Nursing
• PA: Accredited PA degree program

Certifications:

• NP: AANP or ANCC certified (Family, Adult, Gerontology, or Adult-Gerontology Acute Care)
• PA: NCCPA certification

DEA license & Prescriptive Authority

Computer literacy for EMR documentation

COVID-19 vaccination required
