Job Title: Advanced Practice Clinician - Nurse Practitioner/Physician Assistant Program: Senior Community Care (ISNP)

Location: Jefferson County, traveling between two facilities:

• Carthage Center for Rehab
• Samaritan Keep Nursing Home
  
  

Hours: Full-time (40 hours per week), Monday-Friday, 8:00 AM - 5:00 PM Duration: Approximately 4-7 months

About the Role The Senior Community Care (SCC) program delivers integrated, longitudinal care for high-risk health plan members in facility-based settings. As an Advanced Practice Clinician (APC), you will play a vital role as part of an interdisciplinary team, focused on providing exceptional clinical care to senior living residents.

Key Responsibilities

• Conduct comprehensive health assessments
• Manage acute and chronic medical/behavioral conditions
• Document conditions accurately, including ICD-10 coding
• Collaborate with interdisciplinary care teams
• Lead advanced illness and care planning discussions
• Educate and counsel patients and caregivers
  
  

Required Qualifications

• Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license
• NP: Board certification (Family, Adult, Gerontology, or Adult-Gerontology Acute Care)
• PA: Board certification by NCCPA
• Current DEA license/prescriptive authority
• Ability to meet physical demands (e.g., lifting 30 lbs, navigating stairs)
• Reliable transportation
  
  

Preferred Qualifications

• 1+ year of clinical experience (long-term care preferred)
• Familiarity with geriatrics, chronic illness, and advanced care planning
• Proficient in electronic and written medical documentation

• Dress Code: Medical attire (scrub color not specified)
• Travel: Required between facilities during shift
