One of our client is seeking for a Family Nurse Practitioner/Physician Assistant
Type: Perm
Schedule: Mon-Fri, 9am- 5pm

Description
The Family Nurse Practitioner/Physician Assistant insures delivery of comprehensive health care in accordance with the philosophy, standard of practice, objectives, and policies of company. The Nurse Practitioner is responsible for diagnosis and treatment of common and chronic illness focusing on health promotion and disease prevention activities including patient education and counseling in the facility.
Responsibilities And Duties
Salary: $130,000 - 10K Signing Bonus - FNP/PA - $500 per year External experience including FNP / PA / RN experience (capped at 15 years)

Responsibilities And Duties
- Clinical/Technical Responsibilities
- Perform health assessments, physical exams, and order/interprets diagnostic tests.
- Develop and manage care plans, prescribe medications, and monitor patient progress.
- Collaborate with interdisciplinary teams and refer to specialists as needed.
- Educate patients and families on prevention, treatment, and available resources.
- Conduct home/hospital visits and provide age-appropriate care.
- Document all patient care accurately and provide feedback on clinical operations.
- Provide med-phone coverage and participate in health promotion efforts.
- Organizational & Managerial Duties
- Demonstrate reliability, professionalism, and flexibility in daily operations.
- Maintain confidentiality and comply with healthcare system policies.
- Contribute to performance improvement and special projects.
- Support department goals through effective time/resource management.
- Educational & Professional Development
- Participate in training, mentorship, and continuing education.
- Maintain licensure and collaborative agreements; attend relevant workshops.
- Precept students/staff and support team development through feedback and leadership.
- Identify and meet personal learning and professional growth goals.
- Communication & Interpersonal Skills
- Communicate respectfully with patients, families, and staff.
- Promote a positive, professional image and effective collaboration.
- Serve as liaison with community agencies and contribute to care team dynamics.
- Follow policies, relay information clearly, and support a team-based care approach.
- Bachelor's degree and Master's degree in Nursing
- Graduate of an accredited Nurse Practitioner program
- Bilingual, Spanish preferred
- Current NY State Registered Nurse License and Registration.
- Nurse Practitioner Certification and Registration.
- Current Practice Agreement.
- Certified or eligible for certification by recognized specialty organization.
- Current BCLS, Infection Control, Child Abuse
