Job Title: Nurse Practitioner

Overview: Park Springs is seeking a dedicated and compassionate Nurse Practitioner (NP) to join our healthcare team. The NP will be responsible for providing high-quality care to residents through the Wellness Clinic, conducting home visits, and supporting members in our Assisted Living facility. This role requires a skilled clinician with the ability to assess, diagnose, and treat a wide range of health conditions, while promoting wellness and a holistic approach to healthcare.

Key Responsibilities:

Wellness Clinic:

• Provide primary care services, including health assessments, screenings, and preventive care for Park Springs members.
• Develop and implement personalized care plans based on individual health needs.
• Collaborate with other healthcare providers to ensure comprehensive care for all members.

Home Visits:

• Conduct home visits to assess and manage chronic conditions, monitor health progress, and provide education to residents and their families.
• Ensure continuity of care through effective follow-up and coordination with the broader healthcare team. Assisted Living Care:
• Deliver routine and urgent care to members in the Assisted Living community.
• Monitor and evaluate the health status of residents, addressing any concerns and coordinating care with specialists as needed.
• Administer medications, perform medical procedures, and provide ongoing support to the Assisted Living team. Collaboration and Communication:
• Work closely with Park Springs’ interdisciplinary healthcare team, including physicians, nurses, and support staff, to ensure comprehensive care.
• Maintain accurate and up-to-date patient records using electronic health systems.
• Educate residents, families, and staff on health promotion, disease prevention, and treatment options. Compliance and Documentation:
• Ensure compliance with all healthcare regulations, including state and federal guidelines.
• Complete thorough documentation of all patient interactions, diagnoses, treatments, and outcomes in accordance with Park Springs’ policies and procedures.

Qualifications:

• Master’s Degree in Nursing (MSN) and current Nurse Practitioner licensure in Georgia.

• Board certification in Family Medicine, Geriatrics, or another relevant specialty preferred.

• Minimum of 2-3 years of experience in a clinical setting, preferably with geriatric or long-term care experience.

• Strong clinical assessment, diagnostic, and problem-solving skills.

• Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and staff.

• Knowledge of electronic health records (EHR) and healthcare management systems.

• Ability to work independently and as part of a team in a fast-paced environment.

Physical Requirements:

• Ability to travel between facilities and conduct home visits as required.

• Must be able to lift and assist patients as needed, consistent with healthcare standards.
