Join our team as a Nurse Practitioner in Jefferson County, NY, supporting a specialized care program for high-risk patients in skilled nursing facilities.

Key Responsibilities:

- Conduct comprehensive clinical assessments

- Manage acute and chronic medical and behavioral conditions

- Document diagnoses and care plans accurately (ICD-10)

- Collaborate with care teams

- Lead advanced illness and care planning discussions

- Offer education and counseling to patients and caregivers

Requirements:

- Active NP license

- Graduate of an accredited NP (MSN/DNP)

- Board certification (AANP or ANCC)

- Active DEA license and prescriptive authority

- Ability to perform physical assessments and lift 30 lbs.

Preferred Qualifications:

- 1+ year of clinical experience (long-term care preferred)
