Full-Time | Mon–Fri | 8 AM – 5 PM

Approx. 7-Month Assignment

W2 Hourly Paid Weekly.

Overview

Seeking a Nurse Practitioner or Physician Assistant to provide integrated care to high-risk patients in long-term care settings. Enjoy a flexible schedule with no on-call, weekends, or holidays .

Key Duties

• Conduct patient assessments
• Manage chronic and acute conditions
• Document using ICD-10
• Collaborate with care teams
• Lead care planning and patient education
  
  

Requirements:

• Active NP or PA license & national certification
• Graduate of accredited NP/PA program
• DEA license & COVID-19 vaccination
• Able to lift 30 lbs and travel between facilities
• Reliable transportation
  
  

Preferred:

• 1+ year clinical experience (LTC preferred)
• Geriatric and chronic illness knowledge
• EMR proficiency
  
  

Travel:

• 5 facilities across 3 counties
• Travel during shift hours
  
  

Interview:

• Conducted via Microsoft Teams
