We are seeking a compassionate and experienced Nurse Practitioner (NP) to join our growing team. The ideal candidate will have a strong background in geriatric care, excellent communication skills, and a commitment to patient-centered, in-home care. As a key member of our team, you will provide comprehensive medical services, support care coordination, and promote caregiver involvement to enhance quality of life for our elderly patients. The ideal candidate must have at least 2 year-experience and is willing to travel between facilities and homes and can work from Monday to Friday.
