Are you a Nurse Practitioner or Physician Assistant looking for a rewarding opportunity in Huntington, Wabash, and Wells Counties, IN? We have a full-time position available for an approximately 7-month assignment, Monday to Friday from 8 AM to 5 PM.

Overview:

Join us in providing integrated care to high-risk patients in long-term care settings. Enjoy a flexible schedule without on-call duties, weekends, or holidays.

Key Duties:

- Conduct patient assessments

- Manage chronic and acute conditions

- Document using ICD-10

- Collaborate with care teams

- Lead care planning and patient education

Requirements:

- Active NP or PA license & national certification

- Graduate of accredited NP/PA program

- DEA license & COVID-19 vaccination

- Ability to lift 30 lbs. and travel between facilities

- Reliable transportation

Preferred Skills:

- 1+ year clinical experience (LTC preferred)

- Knowledge in geriatrics and chronic illnesses

- Proficiency in EMR systems

Travel:

You will visit 5 facilities across 3 counties during your shift hours.
