The Judge Group is currently seeking Nurse Practitioners/Physician Assistant in Jefferson County, New York to conduct Health Assessments with one of our largest healthcare providers in the country!

Job Scope:

Position: Nurse Practitioner/Physician Assistant

Location: Jefferson County, New York

Contract: W2 Contract Role (4-7 months possible extension)

Shifts: Monday to Friday 8AM-5PM *NO ON CALL, NO WEEKENDS, and NO HOLIDAYS REQUIRED*

Job Description:

• Conduct comprehensive assessments
• Manage acute and chronic medical and behavioral conditions effectively
• Ensure accurate and complete documentation, including ICD 10 conditions
• Collaborate and communicate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide counseling and education to patients and caregivers

Qualifications:

• Family, Adult, Gerontology Nurse Practitioner background required
• Candidates must be board certified by the ANCC or AANP
• Must have active/unrestricted New York NP license
• 1+ year of clinical experience in long term care setting preferred
