Optum is a clinician-led care organization, that is creating a seamless health journey for patients across the care continuum. As a member of our Landmark team, you will help bring home-based medical care to complex, chronic patients. This life-changing work helps give older adults more days at home.

Position Responsibilities

• Provide home-based medical care to complex, chronic patients
• Conduct initial and ongoing patient assessments
• Develop and update care plans
• Perform basic clinical procedures (e.g., IVs, wound care)
• Conduct post-discharge visits
• Participate in quality initiatives and after-hours on-call rotation

Qualifications

• Certified NP or PA (AANP, ANCC, or NCCPA)
• Active, unrestricted NP/PA and RN (for NPs) licenses in WA
• Reliable transportation

Preferred:

• 2+ years of clinical experience (e.g., geriatrics, urgent care)
• Familiarity with long-term care, Medicare/Medicaid, and clinical coding

Washington Residents Only: The hourly range for this role is $42.31 to $83.27 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
