Responsibilities:

Perform Adult/Geriatric Assessments

Manage medical history, diagnoses, and treatments

Offer health education, referrals, and case management

Collaborate with medical staff

Qualifications:

3 years patient care experience preferred (primary care/adult/geriatric,)

Master’s degree required

Board certification by AANP or ANCC
