We are seeking a passionate and dedicated Primary Care Nurse Practitioner who is ready to make a significant impact on the lives of their patients. This permanent position will give you the opportunity to provide comprehensive primary care services to a diverse patient population. You will be working in a dynamic and supportive environment where your expertise in managing chronic illnesses will be highly valued. Responsibilities

As a Primary Care Nurse Practitioner, your primary responsibility will be to provide exceptional patient care. Your duties will include, but are not limited to

• Providing comprehensive primary care services, including acute and chronic illness management, preventive care, and health promotion.
• Evaluating patients' health, diagnosing and treating illnesses, injuries and chronic conditions.
• Managing patients with chronic illnesses, adjusting treatment plans as necessary, and coordinating care with other healthcare professionals.
• Educating patients and their families on disease prevention and healthy lifestyle habits.
• Developing and implementing patient management policies and procedures.
• Participating in quality improvement, management, continuing education, and patient care programs.
• Assisting with the training and supervision of nursing staff and students.
  
  

Qualifications

The ideal candidate for this position is a highly skilled, compassionate, and dedicated professional. The following qualifications are required for this role

• Master’s degree in nursing from an accredited nursing program.
• Current licensure as a Nurse Practitioner in the state of practice.
• A minimum of 1 years of experience as a Nurse Practitioner, with a focus on primary care and chronic illness management.
• Exceptional clinical skills with an emphasis on diagnosing and managing a comprehensive range of health conditions.
• Excellent communication skills, with the ability to effectively communicate complex medical information to patients and their families.
• Strong organizational skills and the ability to manage a high-volume workload.
• Demonstrated commitment to patient-centered care and the ability to build strong relationships with patients.
• Current CPR or BLS certification.
• Ability to work independently, as well as part of a multidisciplinary team.
