Nurse Practitioner or Physician Assistant - In Home Primary Care in Washington

As a Nurse Practitioner or Physician Assistant, you will be a vital member of our interdisciplinary Field Team, delivering home-based primary care to enhance the quality of life for our patients in Washington.

Job Details

• Start Date: June 2, 2025
• End Date: August 30, 2025
• Employment Type: Local
• Hours Per Week: 40
• Shift: 8 Hour Days, 5 shifts per week
• Salary: $130,000 to $145,000 annually plus bonus

Job Requirements

• Graduation from an accredited health professions school as a Nurse Practitioner or Physician Assistant
• Active national board certification
• Active licensure in the state of Washington
• Active DEA licensure and state-specific controlled substance registration, as applicable
• Certification in basic life support (BLS)
• Ability to travel as needed; proof of liability and property damage insurance on vehicle used
• Driver requirements: Licensed for a minimum of 5 years, no vehicle-related suspensions/reinstatements, DUI, reckless driving, or felony convictions within the last 7 years

Responsibilities

• Deliver clinical care flexibly, in-person at patient’s home or virtually
• Provide compassionate, individualized, and holistic patient care including comprehensive primary care, urgent care, and chronic condition management
• Collaborate with our multidisciplinary care team for transitions of care support and complex care management
• Engage in professional development opportunities through clinical education programming
• Participate in after-hours on-call coverage as needed
