NPs will cover 5-6 SNFs within a 30-mile commute from the city center.

Pride Health is hiring a Nurse Practitioner/ Physician Assistant to support our client at a medical facility in Marion, IN. This is a 30-week contract-to-hire offering an excellent opportunity to begin working with a leading healthcare organization

Location: Crawfordsville, IN

Job Duration: 30 weeks (Contract to hire)

Shifts: Mon–Fri, Day Shift

Hourly Pay: - $75 - $82/hour

Primary Responsibilities:

1. Conduct comprehensive assessments
2. Effectively manage acute and chronic medical and behavioral conditions.
3. Ensure accurate and complete documentation, including ICD-10 conditions.
4. Communicate and collaborate with the interdisciplinary care team.
5. Conduct advanced illness and advanced care planning conversations.
6. Provide patients and caregivers with counseling and education.

Must have: Education/Qualifications:

• Active Nurse Practitioner or Physician Assistant certification through a national board.
• NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• PA: Graduate of an accredited Physician Assistant degree program.
• PA: Board certified by the National Commission on Certification of Physician Assistants
• Current, active DEA licensure/prescriptive authority.

Preferred Qualifications:

• 1+ year of clinical experience in practice (long-term care setting preferred).
• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs.
• Understanding of Geriatrics and Chronic Illness.
• Understanding of Advanced Illness and end-of-life discussions.
