Full-Time | Competitive Pay |35 Weeks Contract with Potential of Conversion to Permanent

Location: Watertown, NY & Carthage, NY

NPs will cover SNFs located within a 20-mile commute from the city center.

• Shift: 8 hrs. days Shift, 8 am -5 pm (Mon to Fri)
• Scheduled Hours: 40 Hours a Week.

Gross weekly: $2,600 - $2,800

Key Responsibilities:

• Perform in-depth, comprehensive assessments for senior patients
• Manage and coordinate care for acute and chronic medical and behavioral health conditions
• Accurately document patient information, including ICD-10 coding
• Participate in and facilitate advanced illness and end-of-life care conversations
• Provide education and counseling to patients and caregivers
• Collaborate with interdisciplinary teams to create and follow care plans
• Travel to multiple skilled nursing facilities within designated counties
• Ensure compliance with infection control and safety protocols

Qualifications:

• Active and unrestricted NP or PA license (or ability to obtain by start)
• Graduate of an accredited NP (MSN/DNP) or PA program
• National board certification (AANP, ANCC, or NCCPA)
• Active DEA license with prescribing rights
• Fully vaccinated for COVID-19
