Title: Family Nurse Practitioner/Physician Assistant

Location: Mid-Hudson Valley Region (Onsite)

Employment Type: Full Time, Day Shift, Mon-Fri, 9am-5pm

Compensation

Pay Range: $91000K-$130000K

Description

Ensure delivery of comprehensive health care following standard practices and policies.

Focus on diagnosis and treatment of common and chronic illnesses.

Emphasize health promotion and disease prevention through patient education and counseling.

Work collaboratively with interdisciplinary teams and specialists.

Provide age-appropriate care, including home and hospital visits.

Document patient care accurately and participate in clinical operations feedback.

Key Responsibilities

Perform health assessments, physical exams, and order/interprets diagnostic tests.

Develop and manage care plans, prescribe medications, and monitor patient progress.

Educate patients and families on prevention, treatment, and available resources.

Conduct home/hospital visits and provide age-appropriate care.

Document all patient care accurately and provide feedback on clinical operations.

Participate in health promotion efforts and provide med-phone coverage.

Qualifications

Bachelor's and Master's degree in Nursing.

Graduate of an accredited Nurse Practitioner program.

Bilingual, Spanish preferred.

Current State Registered Nurse License and Registration.

Nurse Practitioner Certification and Registration.

Certified or eligible for certification by a recognized specialty organization.

Current BCLS, Infection Control, Child Abuse certifications.
