Position Summary:

We are seeking a compassionate, dedicated, and experienced Family Nurse Practitioner (FNP) to join our team. The FNP will provide comprehensive patient-centered care to individuals and families across the lifespan. This role involves managing acute and chronic conditions, promoting health and wellness, and working collaboratively with our team to deliver high-quality medical services.

Key Responsibilities:

• • Conduct thorough patient assessments, including history-taking, physical examinations, and appropriate diagnostic testing.

  • Diagnose and treat common acute illnesses, injuries, and chronic health conditions.

  • Develop, implement, and evaluate individualized care plans.

  • Prescribe medications and therapies according to state and federal regulations.

  • Provide preventive care including immunizations, health screenings, and patient education.

  • Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure coordinated and effective care.

  • Maintain accurate, detailed, and timely documentation in the electronic medical record (EMR).

  • Promote patient engagement and self-management strategies.

  • Participate in quality improvement initiatives and continuing education activities.
