Nurse Practitioner – Home Health 

Are you a compassionate Nurse Practitioner seeking to make a meaningful impact in the home health setting? We are seeking a dedicated Nurse Practitioner to join a leading Primary Care at Home program in New Jersey. This is a permanent opportunity to deliver high-quality, patient-centered care to individuals in the comfort of their homes.

Position Details

• Schedule: Full-time, 40 hours per week

• Shift: 8-hour days, Monday–Friday, 8:30 AM – 4:30 PM (with some flexibility)

• Territory: Burlington & Camden County

Key Responsibilities

• Conduct home visits with approximately 8 patients per day

• Provide in-clinic visits for up to 10 patients per day

• Deliver comprehensive, patient-centered care for those with chronic illnesses

• Collaborate with patients, families, and the interdisciplinary team to manage symptoms and improve quality of life

• Participate in on-call rotation every 6–7 weeks (phone triage only)

• Complete documentation using Netsmart EMR

• Engage in monthly orientation and ongoing clinical training

Requirements

• Active Nurse Practitioner license in New Jersey

• At least 1 year of Nurse Practitioner experience

• Experience in palliative care, hospice, geriatrics, or long-term care preferred

• Ability to work independently and manage a home visit schedule

• Strong interpersonal and communication skills
