Nurse Practitioner - Bilingual - Primary Care 

Join our dedicated healthcare team as a Nurse Practitioner in a primary care outpatient clinic. We provide exceptional value-based healthcare to Medicare patients, primarily those aged 65 and older. Job Details

• Employment Type: Permanent
• Schedule: Monday - Friday, 8 AM - 5 PM
• Salary: $105,000 annually
• Location: Berks County PA

Job Requirements

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC
• Active, non-probationary state Nurse Practitioner license
• Active DEA license
• US Work Authorization
• At least 1 year of experience in a primary care setting
• Bilingual proficiency in Spanish is preferred

Responsibilities

• Provide exceptional primary care to patients
• Conduct office visits for routine and acute issues
• Administer annual wellness visits and health risk assessments
• Coordinate care with other providers, specialists, and facilities
• Lead population health initiatives in coordination with the Care Team
• Assist with phone triage and outreach
• Educate patients on health conditions, care plans, and treatments
• Participate in promotional activities and conduct home visits as needed
