Position:  Advanced Practice Provider Nephrology (Nurse Practitioner/Physician Assistant)

Reports to:  Lead MLP & Executive Director/Executive Administrator/CEO                           


Advanced Practice Provider Responsibilities

 

General Responsibilities

  • Provides direct patient care, assessing patients, formulating medical decisions, implementing care plans, and educating patients/caregivers.
  • Completes all necessary education to ensure he/she maintains current knowledge regarding disease management.
  • Supports office, dialysis, and hospital staff by answering questions and providing guidance as needed.
  • Reviews clinical protocols, discussing and recommending revisions as appropriate.
  • Documents all patient interactions adequately and timely, complying with all rules and regulations.
  • Demonstrates the highest level of professionalism, understanding that he/she is representing SNNC at all times, and is regarded as a leader within the healthcare community.
  • Sets an example of quality, competency, and collegiality within the business and throughout the community.
  • Maintains strict confidentiality per HIPAA regulations and adheres to applicable ethics policies and codes of conduct for every facility in which he/she provides patient services.
  • Complies with all local, state, and federal regulations at all times remaining accountable to professional boards.
  • Adheres to all company policies as outlined in the Employee Handbook, MLP Protocols, or elsewhere.
  • Fosters a collegial environment giving and receiving feedback constructively and respectfully.

 

Dialysis Responsibilities

  • Sees dialysis patients monthly as assigned and as needed.
  • Collaborates with Medical Directors and Supervising Physicians regularly for feedback regarding individual cases, treatment plans, and facility/professional goals and opportunities
  • Works with dialysis unit staff to ensure appropriate treatment plans, effective communication, and quality care.
  • Educates patients and staff about dialysis process, modalities, and other clinically relevant information.
  • Acts as the ‘first call’ for dialysis staff’s clinical questions and notifications during facility hours.

 

Clinic/CKD Responsibilities

  • Provides direct patient care in the clinic setting to patients with various renal related impairments, consulting with physician staff as needed.
  • Provides patients with dialysis modality education including transplant and conservative treatments options.
  • Conducts advanced care planning with patients and caregivers as needed.
  • Initiates post-hospital care to ensure effective transitions and reduce re-hospitalization.

 

Hospital Rounding Responsibilities

  • Conducts follow-up rounds with physician support at all hospital campuses, Monday – Sunday as assigned.
  • Works closely with hospital team to ensure daily assignment completion in the most effective manner possible, maintaining high quality care.
  • Communicates directly with hospitalists, other specialists, and hospital staff to effectively coordinate care.
  • Ensures effective documentation for interdisciplinary communication and per hospital policy.

 

 

Minimum Qualifications

 

Skills/Characteristics:

A successful Advanced Practice Provider must be competent in practice with excellent customer service, conflict resolution, and teaching skills.  He/She must also possess superior problem-solving skills, navigating multiple barriers daily.  This position requires mental agility, including the ability to transition between tasks, roles, and responsibilities several times during a single shift.  He/She must be capable of working independently, but within the appropriate scope of practice at all times.  The Advanced Practice Provider must possess strong organizational skills with excellent verbal and written communication, demonstrating superior professionalism throughout his/her performance.


Experience:

New graduates eligible with 5 years of clinical experience required. 

 

Education:

Master’s Degree from an accredited university with a current, unrestricted, state license to practice as an Advanced Practice Registered Nurse or Physician Assistant in Nevada.  Current Nevada Driver’s License and BLS also required.

 

Physical Requirements:

This job often requires travel between multiple worksites within a single day with long periods of walking, standing, and some bending/stooping. 

 

90% - Direct patient care, including documentation, teaching, and care coordination.

10% - Administrative collaboration, teaching, and other duties as assigned
