Join our innovative healthcare team as a Nurse Practitioner specializing in primary care for Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD) patients. This role offers a unique opportunity to work in newly developed outpatient CKD clinics adjacent to dialysis units, providing proactive, comprehensive care.

Job Details

• Location:  Lincoln, NE
• Start Date: April 25, 2025
• Duration: 26 weeks
• Shift: 8-hour days, Monday to Friday
• Shifts per Week: 5
• Employment Type: Full-Time, Local

Job Requirements

• Minimum of two (2) years' experience as a Registered Nurse Practitioner
• Current unrestricted NP license to practice in the state
• Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire
• Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
• Intermediate computer skills and proficiency in MS Word, Excel & Outlook

Preferred Qualifications

• Experience in Value Based Care or Population Health
• 5+ years’ experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case Management
• Spanish-speaking preferred in some markets
• Experience in managing chronic diseases (CHF, diabetes, hypertension)

Responsibilities

• Manage complex patients, focused on reducing hospitalizations and improving health outcomes
• Provide primary care directly in nephrologist offices and dialysis centers
• Assist patients in navigating a challenging healthcare system
• Deliver holistic, integrated care to improve the quality of life for kidney patients
• Work autonomously and in collaboration with all members of the healthcare team
