Position Summary
Performs in-person, no-cost annual preventive health risk assessments for Medicare, Medicaid, and Affordable Care Act members of designated health plans. Assessments take place within members’ personal living spaces, following health plan market expectations.

Performs in-person, no-cost annual preventive health risk assessments for Medicare, Medicaid, and Affordable Care Act members of designated health plans. Assessments take place within members’ personal living spaces, following health plan market expectations.
Hours
- Flexible scheduling (days and evenings)
- 60–75 minutes per appointment
- Market rate per completed assessment
- Mileage reimbursement after the first 30 miles driven per day, based on current IRS rate
Core Responsibilities
- Successfully complete required self-paced and instructor-led training within the allotted time
- Provide a minimum of 30 appointment slots per month
- Conduct high-quality assessments aligned with member satisfaction, health plan expectations, and quality standards
- Submit chart notes within 24 hours of each visit
- Respond to charting queries within 48 hours
- Maintain effective communication with clinical leadership
- Attend periodic training sessions and relevant meetings
- Comfortably enter members’ personal living spaces for appointments
- Lift up to 20 lbs as needed
- Bend or kneel for extended periods if necessary during assessments
Competencies
- Proficiency in electronic medical records and strong computer skills
- Knowledge of CMS and ACA risk adjustment models
- Accurate and thorough documentation of assessments
- Strong verbal and written communication with members and clinical teams
- Compliance with HIPAA regulations and confidentiality standards
- Critical decision-making following established policies and procedures
- Adaptability to changing work responsibilities and environments
- Positive attitude and collaborative approach with members and colleagues
- Proactive problem-solving for member, provider, and health plan concerns
- Master's or Doctorate degree from an accredited Nurse Practitioner (NP) program (FNP, AGNP, ANP)
- Master's degree from an accredited Physician Assistant (PA) program
- Minimum 5 years of recent home health experience (required)
- 1+ year of in-home assessment experience (highly preferred)
- Current RN and NP or PA license in the practicing state
- Board certification by ANCC, AANP, or NCCPA
- Active Medicaid ID number for the state
- Active Medicare PTAN number
- Current BLS certification
- Valid and unencumbered driver’s license
- Proof of sufficient auto insurance coverage for scheduled appointments
