Duties Description
The Nurse Practitioner will perform the following general duties: Performs initial medical screening on assigned patients or when consulted; Performs ongoing assessment to determine how the needs of the patient/family/caregiver will be met. Monitors treatment, progress and medications of assigned patients. Documents as per CMS regulations. Participates in weekly rounds on assigned patients, Orders medications, consultations, laboratory and radiology requests in accordance with hospital guidelines. Ensures the patient/family/caregiver is aware of medical recommendations/follow-up. Completes Discharge Summary which reflects status and plan of care post discharge. Other duties as assigned.

Reassignment: Current New York State Department of Health Helen Hayes Hospital employee with permanent or contingent permanent status as a Nurse Practitioner (Family Health).
HELPS: Must posses certification and current registration as a Nurse Practitioner in a specialty practice area by the New York State Education Department.
Current New York State employee with one or more years of permanent, contingent-permanent, or 55 b/c service that is eligible for transfer under Section 70.1 of the Civil Service Law to Nurse Practitioner (Family Health).
Preferred Qualifications: Prior practice in family health

The posted salary $159, 252 - $159,252 includes statutory hiring rate at step 4 and job rate for PS&T Grade 24 ($91,298 - $115,252), plus an additional geographic pay differential of $40,000 and downstate adjustment of $4,000.

For new State employees appointed to graded positions, the annual salary is the hiring rate (beginning of the Salary Range) of the position.
