What You’ll Do
As a Nurse Practitioner at CleanSlate, you’ll play a vital clinical role in treating patients with opioid and alcohol use disorders through medication-assisted treatment (MAT). You’ll work alongside a team of dedicated medical and behavioral health professionals to ensure high-quality, patient-centered care.

What You’ll Do
As a Nurse Practitioner at CleanSlate, you’ll play a vital clinical role in treating patients with opioid and alcohol use disorders through medication-assisted treatment (MAT). You’ll work alongside a team of dedicated medical and behavioral health professionals to ensure high-quality, patient-centered care.
Key Responsibilities
- Provide medication-assisted treatment using buprenorphine, Vivitrol, Sublocade, and Brixadi
- Evaluate patients and determine individualized treatment plans
- Monitor patient progress and adjust medications as needed
- Collaborate with physicians and care teams on complex cases
- Maintain accurate documentation in the EMR within 48 hours
- Participate in clinical meetings, training, and quality improvement efforts
- Support telemedicine and occasional regional coverage as needed
- Active NP license and DEA registration in state of employment
- Graduate of an accredited Nurse Practitioner program
- Strong interpersonal and critical thinking skills
- Experience in addiction medicine or behavioral health preferred
- Comfort with EMRs and a fast-paced clinical setting
- Monday–Friday schedule; No Weekends, No Night, No On-call
