Title: Nurse Practitioner Supervisor

Compensation

Pay Range: $155000K-$165000K

Description

Collaborate with primary care physicians to ensure patient care coordination.

Gather patient histories, perform physical exams, and conduct diagnostic tests.

Diagnose and treat common medical conditions within scope of practice.

Manage chronic illnesses and tailor treatment plans accordingly.

Educate patients and families about disease prevention and health promotion.

Document patient records in electronic health records accurately.

Participate in quality improvement initiatives to enhance patient care.

Key Responsibilities

See adult patients and serve a diverse community.

Perform annual wellness and routine quarterly assessments.

Provide acute care services and facilitate same-day access.

Manage participants with acute changes in condition timely.

Conduct post-hospitalization visits and coordinate care transitions.

Supervise care plan formulation and delivery for community participants.

Ensure documentation is complete and accurate with specific diagnosis codes.

Qualifications

Graduated from an accredited nurse practitioner program.

Minimum two years of clinical nursing practice in a primary care setting.

One year of experience working with adult population or appropriate training.

Licensed as a Nurse Practitioner and Registered Nurse in NY.

DEA Certification and Board Certification required.

Effective communication and interpersonal skills.

Basic computer skills in office software.
