Job Title: Advanced Practice Clinician – Nurse Practitioner / Physician Assistant

Schedule: Monday–Friday, 8am–5pm (40 hours/week)

Duration: 3–4 months (FMLA coverage starting 8/1)

Interview Format: Microsoft Teams (2 rounds)

Dress Code: Medical attire (no specific scrub color)

Role Overview:

Provide integrated, longitudinal care to high-risk patients in senior living facilities as part of an interdisciplinary team. This role offers flexible scheduling with no on-call, weekends, or holidays.

Key Responsibilities:

• Conduct comprehensive assessments
• Manage acute and chronic conditions
• Document care accurately (ICD-10)
• Collaborate with care teams
• Lead advanced illness and care planning discussions
• Educate patients and caregivers

Required Qualifications:

• Active NP or PA license (or ability to obtain)
• NP: MSN or DNP; PA: Accredited PA program
• Board certification (AANP, ANCC, or NCCPA)
• DEA license and prescriptive authority
• Ability to travel between 3 facilities in Guilford County, NC
• Physical ability to lift, kneel, and navigate varied environments
• Reliable transportation
• COVID-19 vaccination required
• Drug screening required

Preferred Qualifications:

• 1+ year clinical experience (long-term care preferred)
• Experience with geriatrics, chronic illness, and end-of-life care
• Strong documentation and EMR skills
