Role Overview 

As a Primary Care Nurse Practitioner in a psychiatric setting, you’ll provide high-quality, patient-centered care to residents with chronic conditions, geriatric needs, and complex comorbidities. You’ll collaborate with interdisciplinary teams to develop and implement personalized care plans, monitor patient progress, and respond to acute changes in condition.

Key Responsibilities 

• Conduct in-person clinical visits and routine assessments
• Diagnose and manage chronic conditions (e.g., diabetes, hypertension, arthritis)
• Develop and adjust individualized care plans
• Collaborate with physicians, nurses, and rehab staff
• Educate patients and families on health management
• Document encounters using EHR systems with CPT/ICD-10 coding1
• Participate in case management and discharge planning
• Ensure compliance with state and federal healthcare regulations

Qualifications

• Current Nurse Practitioner license in New York
• Active DEA License
• 2+ years of clinical experience in geriatrics
• Comfortability providing primary care to psychiatric patients
• BLS and ACLS certifications
• Familiarity with electronic documentation platforms
• Compassionate, patient-focused approach
